WAIVER OF LIABILITY, ACKNOWLEDGMENT OF RISK & INFORMED CONSENT

Please read carefully and check each box to confirm your understanding before participating in any dreamlike4paws/ Proactive Canine Solutions (Event Organizers) class or event.

I understand that participation in any Event Organizers activity, including Mantrailing classes, workshops, or events (the Event), involves inherent risks. These may include, but are not limited to, the unpredictable behavior of dogs and other animals, potential injury, serious harm, or death to people or animals, and possible property damage. I acknowledge that even well-trained dogs can behave unpredictably and that no guarantees can be made regarding the behavior of any dog, including my own

I understand that training environments and scent work setups may include varied surfaces, obstacles, or footing challenges that could present safety risks. I agree to take reasonable precautions for my safety and my dog’s safety and to follow all instructor directions and class safety guidelines at all times.

I voluntarily assume all risks associated with participating in, observing, or volunteering at any Event Organizers class or event. I understand that injuries, illness, property damage, or other losses can occur and that I take full responsibility for myself, my dog, and anyone attending with me.

I agree to release, indemnify, and hold harmless dreamlike4paws or Proactive Canine Solutions, its owner, employees, and contractors from any and all claims or expenses related to injury, illness, death, or property damage, except in cases of proven gross negligence or intentional misconduct. I also agree to be responsible for any damages or claims arising from the actions of myself, my guests, or my dog.

I confirm that my dog will not wear or use aversive tools such as shock collars, prong collars, or choke chains during class.

PHOTO AND VIDEO RELEASE

I understand that education and public relations are an important part of participating in Mantrailing activities. I give the Event Organizers permission to use photographs, videos, or audio recordings of me and my dog for educational, promotional, or training purposes. I agree that these materials may be used in print, digital, or online formats, including websites and social media, for public relations, marketing, or educational content.

Health Certification

I certify that my dog is up-to-date on all required vaccinations, including rabies, and is free of contagious illnesses. I understand that I may be asked to provide proof of vaccination upon request.

I agree to notify the Event Organizers if my dog comes into season (heat) before attending any scheduled class session. I understand that in-season females may not be permitted to attend class unless prior arrangements have been discussed and approved.

CANCELLATION AND SCHEDULING POLICY

I may cancel a class reservation up to 7 days prior to the scheduled class start free of charge. After that, cancellations will be reviewed on a case-by-case basis at the discretion of the Event Organizers.

Refunds for missed classes will not be issued unless explicitly approved by the Event Organizers.

I understand that failure to follow these policies may result in my removal from the Event without a refund.
